To become a certified provider to CRI, Inc:
- Supply your updated Vendor Contact Information: Company Name, Tax ID, Remittance Address, Phone, Fax, Email, Contact, Vendor Type, etc.
- Read and Accept CRI, Inc. Agreement
- Provide the name and address for each of your company’s principals for Terrorist Watchlist screening required by Federal Law
- Provide your license number and state issue for any professional license
- Print and email a signed copy of an IRS W9 Form
- Onsite vendors please have your producer send us a copy of your Insurance Certificat for the following items: GENERAL LIABILITY, AUTO, and WORKERS COMP coverage
- Offsite vendors please have your producer send us a copy of your Insurance Certificate for the following items: GENERAL LIABILITY and ERRORS AND OMMISSION coverage
- Producer Information: please have ready the phone/fax/email for each producer [insurance agent] that currently handles your insurance
